Referring a patient

How our referrals process works

Complete the referral form and submit it with the patient’s insurance card, photo ID, demographics (including SSN), relevant labs or imaging, and the most recent encounter notes.

Once received, charts are created within 1–2 weeks. STAT referrals are scheduled within one week. Our team makes three attempts to reach the patient, and routine appointments are typically scheduled 4–6 weeks out.

After scheduling, a confirmation with the patient’s consult date and time is faxed to the referring provider. If the patient cannot be reached or scheduled, a notification explaining the reason will be sent.
